Position Purpose:
Food and food service delivery Drivers fulfill a critical role in the country’s food supply chain. Our delivery drivers not only make sure the customers’ products arrive at their destination at the arranged times and in good condition, but they are the face of our company – building lasting relationships with our customers!

The Driver, Hourly CDL-A/B is responsible for driving a tractor trailer or straight truck on intrastate and interstate local, over-the-road (OTR), shuttle, and overnight and drive and drop routes for the purpose of delivering and/or unloading food and food related products to customers in a safe and timely manner and in accordance with Department of Transportation (DOT) regulations. Communicates and interacts with customers, vendors and co-workers professionally ensuring questions are answered accurately and in a timely manner. Functions as a team member within the department and organization, as required, and perform any duty assigned to best serve the company.

Responsibilities may include, but not limited to:
  • Performs all required safety checks (i.e., pre/post trip) including inspections of tractor/truck and trailer according to Department of Transportation (DOT) regulations; inspects tractor/truck and trailer to insure they meet company safety standards and take appropriate action as needed. Reports all safety issues and/or repairs required.
  • Follows all DOT regulations and company safe driving guidelines and policies.
  • Immediately reports all safety hazards. Inspects trailer for properly loaded and secured freight.
  • Performs count check of items and check customer invoices of products that have been loaded. Checks and completes in an accurate and in legible fashion all required paperwork associated with freight.
  • Moves tractor to the loading dock and attach preloaded trailer as needed.
  • Drives to and delivers customer orders according to predetermined route delivery schedule. Unloads products from the trailer, transports items into designated customer storage areas.
  • Performs damage control checks on items, scans, and contact supervisor about removing orders according to company policy. Verifies delivery of items with customer and obtain proper signatures. Collects money (cash or checks) where required.
  • Loads customer returns on to trailer and secures trailer doors. Ensures that tractor, trailer, and freight are appropriately locked and/or always secured.
  • Unloads damaged goods and customer returns and bring to the driver check-in and complete necessary paperwork. Unloads all equipment, materials and remove trash from trailers as required.
  • Completes daily record of hours of service and enter in log in accordance with Federal DOT, state, and company requirements.
  • Performs general housekeeping duties in tractor, loading dock area and keep trailers clear and clean as required. At the end of the shift secure all equipment and complete all necessary paperwork.
  • Performs other related duties as assigned.

Qualifications:

High School Diploma/GED or Equivalent

6+ months commercial driving experience

Valid CDL-A/B

Must be 18+ years of age

Meet all State licensing and/or certification requirements (where applicable)

Clean Motor Vehicle Report (MVR) for past 3 years

Pass post offer drug test and criminal background check

Pass road test

Attains or has valid current DOT Health Card and/or able to secure new DOT Health Card

Able to hand-lift and utilize two-wheeler, lift gate and/or other equipment to move and/or stack product cases/freight of varying size and weight throughout shift; product generally ranges from between 10-35 pounds and up to between approximately 60 and 90 pounds, depending on the location

ESSENTIAL JOB FUNCTIONS

Staff Management: Oversees implementation of human resource practices including recruitment, retention, training, scheduling, processing, record keeping, and management of program employees;

  1. Recruitment & Hiring:
    • Participates in the recruitment and interviewing of all staff;
    • Hires direct staff members in consultation with the PD’s manager; and approves the hiring of direct support professionals and other indirect staff members.
  2. Meetings & Training:
    • Organizes and implements on-site orientation and training for assigned programs; and monitors training compliance and documentation.
    • Conducts monthly staff meetings; attends monthly regional meetings.
  3. Employee Scheduling:
    • Designs and monitors schedules to assure that all shifts are covered;
    • Monitors and minimizes overtime.
  4. Performance Evaluation:
    • Conducts annual performance evaluations for direct reports in consultation with the PD’s manager;
    • Reviews and approves annual Direct Support Professional and other staff member’s performance evaluations.
  5. Coaching, Discipline and Termination:
    • Coaches employees, correcting or directing employee behavior where required;
    • May remove employees in the event of emergency;
    • May recommend and participate in decisions regarding discipline and termination in consultation with the PD’s manager and Human Resources;
    • Manages the termination of Direct Support Professionals or other staff members as necessary;
  6. Employee Safety:
    • Responsible for employee safety and workers’ compensation programs;
    • Monitors compliance; and assists in returning employees to work;
    • Implements, oversees, and trains employees about safety.

Program Management: Manages the implementation of program planning; monitors delivery of services and supports; ensures services are consistent, engaging and relevant to the individuals served.

  1. Program Planning:
    • Acts as the Qualified Intellectual Disabilities Professional (“QIDP”), writes and approves individual program plans;
    • Trains and assists appropriate staff members with implementation of plans across all shifts.
  2. Records: 
    • Periodically audits individual records and other program documentation for accuracy and completion.
  3. Program Team Meetings:
    • Conducts progress meetings and (quarterly, annual and/or other) reviews as appropriate or determined by the interdisciplinary team;
    • Provides and approves written and progress reviews.
  4. Compliance:
    • Oversees licensing compliance for program services and supports;
    • Ensures timely and accurate program documentation;
    • Implements the Network compliance plan; and manages individual rights implementation.
  1. Management of Individuals’ Assets:
    • Implements system and oversees management of individual’s funds and property, assuring safety and accuracy; audits, monitors and reviews individual’s financial accounts. 
  2. Stakeholder Relations:
    • Maintains productive and professional relations with individuals, families, case managers and other stakeholders;
    • Provides regular communication to stakeholders; and acts as point of contact for all stakeholders;
    • Implements satisfaction improvement plans.
  3. Incidents:
    • Implements proactive strategies to reduce or eliminate serious incidents;  
    • Takes immediate steps to correct potential safety concerns and limit the potential for continued concern or incident;
    • Reviews all incidents and directs appropriate response; ensures timely incident reporting; and signs and submits all incident reports as required.

Health Care:  Oversees implementation of individual health care planning; implements strategies to limit illness or serious incident; oversees and reviews health care documentation. 

  1. Appointments:
    • Oversees coordination of health care appointments and monitors individual attendance.
  2. Medications:
    • Oversees system for medication administration, documentation and security;
    • Reviews documentation for accuracy and completeness;
    • Assures all applicable employees are certified or trained in medication administration.
  3. Medical Supplies and Equipment:
    • Approves purchases of medical supplies and equipment; and ensures training on proper use to all employees.
  4. Dietary Planning:
    • Monitors food service in accordance with dietary plans and to meet regulatory compliance;
    • Ensures consideration for special needs, occasions and individual choice.

Finance: Responsible for the financial performance of the program, from billing to staff budgets, as well as reviewing and approving purchases

  1. Purchasing:
    • Approves purchase of food, supplies and equipment; and reviews and monitors monthly invoices for accuracy and budgetary compliance.
    • Attests to company credit card statements and monthly invoices in accordance with company policy.
  2. Billing and utilization:
    • Oversees census, attendance information or other required documentation to assure accurate billing;
    • Monitors utilization between authorization and provision of services and supports.
  3. Staffing:
    • Monitors staffing hours for budgetary compliance.
  4. Growth:
    • Maximizes census, average daily attendance and utilization.

Maintenance: Oversees the maintenance and cleanliness of the program’s facilities and vehicles.

  1. Vehicles:
    • Ensures vehicles are properly maintained, clean, and safe by consulting with maintenance personnel;
    • Implements training procedures for maintenance and safe operation of company-owned vehicles;
    • Addresses known safety concerns with employee vehicles or other means of transportation, for the safe transport of individuals.
  2. Housekeeping:
    • Monitors and implements strategies to assure neat, clean, and safe program environments (internal and external);
    • Ensures age appropriate and esthetic decoration.
  3. Maintenance and Repair:
    • Oversees environmental safety program; establishes preventive maintenance schedule; maintains furnishings, equipment and facilities in good repair and compliant with regulatory standards.

Other:

  • Attends monthly regional meetings and additional meetings as needed
  • Performs other related duties and activities as required. 

SUPERVISORY RESPONSIBILITIES

  • Manages the direct or indirect staff members of the program including performance evaluations, scheduling, orientation, and training. Makes recommendations and/or decisions on employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions. Resolves employee problems within position responsibilities.

  • Monitors and records medications in a timely manner; acts as a liaison to pharmacy.  Reviews medication error forms, analyzes errors and makes recommendations for procedural changes. Administers medication as required.
  • Oversees and monitors staffs who administer medication.  Conducts quality assurance checks on health care records.
  • Provides emergency coverage and/or care for individuals receiving services. 
  • Maintains confidentiality, respects human rights and practices universal precautions in accordance with Mentor policy.  Upholds policy and procedure development related to health care.
  • Works with external agencies to coordinate services and ensure consistent treatment of clients.
  • Administers, tracks, and maintains log for staff and individual’s testing.
  • Provides training for staff including medication administration, universal precautions and infection control.

  1. Marketing: Identifies collateral, social media, virtual tour, and other marketing needs within the area and escalates those needs to the Regional Director.
  2. Referrals: Serves as the primary contact for individuals, families, guardians, and case managers throughout the referral process. Responds to inquiries and referrals promptly and professionally. Coordinates local events, community participation, and engagement with referral partners, including managed care organizations and state referral sources. Manages the intake referral checklist and referral pipeline for the assigned area. Reviews referrals to determine program fit and identifies and escalates potential barriers to the Regional Director.
  3. Census Performance: Monitors census and referral dashboards, evaluates progress toward census targets, and analyzes trends involving admissions, discharges, and leave days.

Growth and Development Strategy

  1. Growth Strategy: Implements the area’s core growth strategy, including census growth, periodic billing, and new service starts, to increase service volume.
  2. New Program Development: Assists with planning and opening new programs, including identifying potential real estate and purchasing furnishings, supplies, and equipment. May also identify potential acquisition opportunities.

Stakeholder Relations

  1. Program Stakeholders: Develops and maintains productive relationships with case managers, referral agents, community partners, and funding sources.
  2. Government Relations: Participates in building relationships with local legislators and government officials. Attends grassroots advocacy meetings, town halls, advocacy days, and other community events. Advocates on behalf of individuals served and Sevita when appropriate.
  3. Customer Service: Implements strategies to build and maintain positive relationships with individuals, mentors, families, and guardians. Develops and implements satisfaction improvement plans as appropriate and consistently upholds Sevita’s customer service standards.

Compliance and Regulatory Management

  1. Quality: Oversees the area’s quality enhancement and implementation plan, ensuring alignment with Sevita standards, licensing requirements, and the needs of individuals served. Reviews area incident reports and quality trends.
  2. Incident Management: Implements area incident management protocols, ensures timely incident reporting and corrective action, and assists with investigations as needed.
  3. Licensing and Compliance: Maintains licensing and regulatory compliance for area services and supports. Ensures documentation is timely and accurate, oversees implementation of Sevita’s compliance plan, and assists with correction orders and related compliance improvement plans.

Health and Safety Management

  1. Health and Safety Programs: Oversees the development and implementation of health and safety programs for individuals served and employees. Supervises area nursing staff when applicable.
  2. Environment: Ensures area programs comply with Sevita and regulatory standards for clean, safe, and well-maintained living and working environments. Promotes pleasant, age-appropriate, and home-like residential settings and oversees area maintenance needs.

Additional Responsibilities

  • Performs other duties and activities as required, including providing coverage or backfilling positions under their supervision when necessary.

Supervisory Responsibilities

Directly manages and supervises Program Supervisors/Coordinators, Direct Care staff, and other assigned personnel. Responsibilities include performance evaluations, scheduling oversight, orientation, training, coaching, and employee development. Makes or recommends decisions regarding employee transfers, promotions, compensation changes, disciplinary action, terminations, and similar employment matters. Addresses and resolves employee concerns within the scope of the position.

SUMMARY
Provides direct care to clients/individuals receiving services.  Coordinates development and implementation of services/treatments provided to clients/individuals.  Trains staff, schedules for shifts, and performs other administrative duties.  Provides oversight and coordination of staff.  Provides effective leadership and direction to staff in the absence of manager/director.   
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:  
•Assists in accessing medical and health-related services for individuals receiving services. Provides on-call coverage for caseload and participates in rotational emergency coverage and crisis intervention support as needed.
•Assesses referred clients and participates in the development of a service plan in conjunction with a treatment team and funding source in accordance with corporate, contractual and regulatory standards.  Coordinates and evaluates plan implementation. 
•Assists in the recruitment, screening, hiring and evaluation of staff.  Resolves employee problems under guidance of manager/director.  May manage staff schedules.
•Implements and actively contributes to development, review and revision of programs and behavior intervention plans. 
•Schedules and/or provides opportunities for quality training, recreation and leisure activities with consideration for choice, preference and age-appropriateness.  
•Provides monitoring, consultation and comprehensive support services to mentors/staff regarding work with individuals supported and implementation of service plans on an ongoing basis. Makes home visits and conducts meetings as required.
•Provides Direct Care in the assigned program(s) on a weekly basis as scheduled.
•Maintains current and complete client and mentor/staff reporting records in accordance with corporate and regulatory standards. Assists with all legal, licensing, regulatory and certification activities as needed.
•Provides pre-service and in-service training to mentors/staff as assigned and assists in mentor training, planning and development.  
•Maintains current and complete client records and reports in accordance with corporate and regulatory standards.
•Performs other related duties and activities as required.
SUPERVISORY RESPONSIBILITIES
May provide oversight and coordination of staff under the direction of assigned manager as required.
Minimum Knowledge and Skills required by the Job
