Position Summary

Pagosa Springs Medical Center (PSMC) is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our Health in Motion Physical Therapy team. We are open to both full-time and part-time candidates and welcome applicants seeking a schedule that provides the right fit while supporting the needs of our patients and department.

Working under the direction of a licensed Physical Therapist, the PTA provides hands-on treatment to patients of all ages recovering from injuries, surgeries, chronic conditions, and other physical impairments. This role plays a vital part in helping patients improve mobility, restore function, reduce pain, and achieve their rehabilitation goals through individualized treatment plans and patient education.

Key Responsibilities

Patient Care

  • Deliver physical therapy treatments under the direction of a licensed Physical Therapist
  • Implement individualized treatment plans designed to improve strength, mobility, balance, endurance, and function
  • Utilize therapeutic exercise, stretching, massage, heat, ice, and other treatment modalities
  • Assist patients with transfers, gait training, balance training, and therapeutic activities
  • Fit patients for braces, prosthetics, walkers, crutches, and other assistive devices
  • Monitor patient response to treatment and report changes to the supervising Physical Therapist

Patient Assessment & Documentation

  • Measure range of motion, vital signs, strength, balance, and functional mobility
  • Accurately document patient progress in the electronic health record
  • Assist with patient intake, medical history, medication reconciliation, and quality reporting
  • Modify treatment interventions as directed by the supervising Physical Therapist
  • Ensure documentation supports quality care and regulatory compliance

Patient Education

Educate patients and families regarding:

  • Home exercise programs
  • Injury prevention
  • Recovery expectations
  • Safe use of assistive devices
  • Treatment goals

Reinforce physician and Physical Therapist recommendations and encourage patient participation and independence throughout the rehabilitation process.

Care Coordination

  • Collaborate with Physical Therapists, providers, nursing staff, and other healthcare professionals
  • Coordinate patient appointments and follow-up care
  • Review referrals, insurance authorizations, and treatment orders
  • Respond to patient phone calls and portal messages appropriately
  • Promote seamless communication throughout the continuum of care

Department Support

  • Prepare treatment rooms and therapy equipment
  • Maintain a clean, organized, and safe therapy environment
  • Monitor and replenish therapy supplies
  • Participate in quality improvement initiatives and continuing education
  • Support departmental workflows and patient-centered care

Qualifications

Education

  • Graduate of a CAPTE-accredited Physical Therapist Assistant program strongly preferred
  • Associate degree preferred
  • Bachelor's degree in Kinesiology, Exercise Science, or a related field will also be considered
  • Successful completion of the National Physical Therapy Examination (NPTE)

Experience

  • Two (2) years of Physical Therapist Assistant experience preferred, but not required

Licensure & Certifications

  • Current Colorado Physical Therapist Assistant (PTA) license
  • Current AHA BLS certification for Healthcare Providers
  • Basic computer proficiency, including Microsoft Office and electronic health records

Skills

Summary

The Scheduling Secretary is responsible for interacting between staff, administration and family in scheduling home health aides in the Home Health & Hospice departments.

Education and/or Experience

High school diploma or GED preferred. Experience as a Nurse Aide preferred. Bi-lingual experience preferred but not required.

Essential Functions

  • Maintains accurate records of monthly and quarterly statistics and patient census for all services.
  • Greets office visitors, acknowledging appointments, offers seating and coffee as appropriate. Alerts appointee. Determines urgency of situations. Decides emergency need to interrupt staff/management staff.
  • Interacts with patients, physicians, third parties, and other departments regarding complaints, problems, etc.

  • Collaborates with Supervisor to coordinate HHA/Homemaker vacations/time-off.
  • Coordinates overall patient and staff scheduling for the department.
  • Completes and distributes monthly calendars for patients.
  • Arranges for weekend and sick-leave coverage for patient visits.
  • Monitors patient and staff satisfaction regarding scheduling and resolves problems and concerns along with Manager.

The DMS Logistics Specialist will help coordinate international and domestic shipping. Oversee teardown activities from operational sites where DMS performs business operations to repair vendors & the DMS warehouse. The successful candidate will be comfortable working in a dynamic, fast moving work environment with changing priorities being tracked at a detailed level. Coordinate projects involving logistics movements of parts by truck, air, and ocean. Track support equipment between sites. Report status of parts movements. Perform shipment reconciliations. Perform system transactions & review data for completeness and accuracy. Responsible for performing a wide range of tasks in support of international trade compliance. The position requires use of SAP, MS Excel, Word, Outlook, SNAP-R, ACE and screening tools. Screen business partners and visitors. Prepare shipping packets and complete all required paperwork for internal and external compliance (BOLs, labels, pick tickets and checklists). Maintain recordkeeping requirements for international shipping. Ship goods and verify items against import/export requirements, where applicable. Coordinate with transportation companies to plan, schedule and manage import and export shipments. Converting all paper records to digital records. Assist in the writing of procedures, work instructions, forms, and training documents for ITC. Any other duties assigned as determined by the Director of Quality or Global Transportation Program Manager.

Quals-- Minimum of 5 years relevant experience. Bachelors degree preferred in Materials Management, Trade Compliance Certification, or related field. Prior experience with import & export clearance procedures, & dangerous goods preferred. Candidate should have the ability to be extremely organized, detail oriented, solve problems and be a self-starter. Ability to work under fast paced environment and prioritize objectives. Ability to properly classify products with HTS and ECCN. Willingness to travel as required. Excellent communication skills both verbal & written. TOC badge required.

Key Responsibilities

  • Design, develop, test, secure, and deliver high-quality backend software for a transaction-processing platform.
  • Design and implement event-driven architectures using patterns such as Event Sourcing and CQRS.
  • Develop domain models and determine when new business patterns require new solutions versus extensions of existing models.
  • Build and maintain secure multi-tenant and multi-party systems with server-side tenant isolation and authorization.
