About the Role

The ideal Family Medicine Physician will offer a personalized approach to healthcare and holistic medical services, treating various conditions across all age groups. You will proactively develop patient interactions and refer patients to outside specialists when care falls outside your medical scope. You will produce healthcare and health management programs based on individual patients' historical data, contributing to the clinic's mission of expanding access to quality care in rural California.

Key Responsibilities

  • Provide primary care to patients of all ages
  • Develop patient-specific health programs using historical data
  • Monitor, adapt, and administer individualized healthcare programs
  • Use patient health data for medical analysis and offer comprehensive wellness exams
  • Administer vaccinations as appropriate
  • Perform medical procedures when necessary
  • Educate patients and families about health management and disease prevention
  • Refer patients to specialists for specific treatment or procedures as needed
  • Collaborate with the interdisciplinary clinical team including the Medical Director and mid-level providers

Requirements

  • Minimum 1 year of experience as a Family Medicine practitioner required
  • Completed Family Medicine internship and residency required
  • Board Certified by the American Board of Family Medicine required
  • Active California medical license in good standing required
  • Active participation in continued medical education (CME) highly preferred
  • Membership in a professional medical organization highly preferred
  • Ability and willingness to work in Newman, CA

PURPOSE: The Resident Support Specialist reports to the Program Supervisor. The Resident Support Specialist provides direct care and educational opportunities while interacting with clients to ensure healthy relationships, positive skills, and provide a supportive living environment. The Residential Support Specialist is also responsible for some administrative work, and DCFS case management notes with data entry.



ESSENTIAL DUTIES AND RESPONSIBILITIES:

  •  Assists in providing trauma-informed, evidence based, and therapeutic services to youth enrolled within transitional residential programs.
  •  Focus on client needs; anticipate, understand, and respond to the needs of clients to meet or exceed their expectations.
  •  Protect residents to ensure physical and emotional safety, to include mandated reporting of any observed or suspected incidents of child abuse or neglect.
  •  Assist program participants so that they are properly clothed and outfitted, engaging in good hygiene practices, and living in a clean, orderly, and safe environment that promotes calm and good health.
  •  Attend and actively participate in staff meetings and job-related training.
  •  Must be able to successfully complete Therapeutic Crisis Intervention (TCI) Training.
  •  Ensure the day-to-day activities of the program are being followed in accordance with the scheduled events, the program rules and the daily staff check list.
  •  Ensure that program schedules and expectations are being always followed.
  •  Report unusual behavior, unplanned visitors, potential conflicts, and conflicts to the program supervisor.
  •  Assists in transportation of residents as needed.
  •  Attend all staff meetings and required training.
  •  Complete all required documentation for daily activities and case notes.
  • Perform additional duties as assigned.

DescriptionSummary:The Registered Vascular Technologist (RVT) performs a range of non-invasive vascular diagnostic procedures to support the assessment and treatment of vascular conditions. Operating under general guidance, the RVT applies specialized clinical knowledge and judgment to produce accurate and high-quality imaging results. We are looking for compassionate Caregivers to be a part of our team and join us in our mission of enhancing seniors’ lives, one day at a time. You work individually with clients, one-on-one, providing a variety of non-medical services that allows you to form relationships with seniors who desire to age in place in their own homes.

  • Starting at $17.00 per hour based on client care experience and certification
  • Higher hourly rate for short shifts

Responsibilities and Duties

With dignity, compassion, and care, your duties include, but not limited to:

  • Perform activities of daily living and provide companionship for our clients in their homes
  • Personal hygiene including but not limited to bathing, grooming and dressing
  • Meal planning and preparation
  • Light housekeeping
  • Provide ambulation and transferring assistance as needed
  • Help with medication as required
  • Transportation for appointments and errands

Qualifications and Skills

  • Must be 18 years old or older
  • Preferred CNA or HHA license or 6 months of full-time work or 12 months of part time work caring for geriatric or chronically ill persons.
  • Must be physically able to perform the duties of this position, including lifting and transferring up to 50 pounds
  • Must have an active CPR Certification or be willing to get certified
  • Valid driver’s license and reliable vehicle with valid insurance
  • Must possess excellent communication skills
  • Are mature, caring, punctual and reliable
  • Must have an active State of Florida Level 2 Background Check or pass one
