General Summary

  • Participate as a clinical expert in payer negotiations and escalations, Accounts Receivable Reconciliation Group (AARGs) and Medical Director Meetings (MDMs), and during Administrative Law Judge (ALJ) hearings for denied cases.
  • Standardize and oversee a comprehensive reliability-driven process for status determination in concert with site level physician advisors.
  • Prepare and analyze denial metrics and make recommendations for process improvements.
  • Provide feedback to system IPAS regarding denial trends, opportunities for improvement, and payor practices.
  • Report out metrics and initiatives at Medical Executive and Executive Leadership meetings as requested.
  • Assist the manager of CDRU, UM Director, and Director of Payor Audit with preparation of case escalations for payer negotiations.
  • Assist the CDRU RN staff with medical necessity appeals for complex medical cases.
  • Maintain a working knowledge of applicable Federal, State and local laws and regulations, initiatives, and expectations as set by payers and CMS.
  • Collaborate with accountable leaders to develop processes that ensure consistent feedback for all involved care providers regarding specific denial mitigation strategies and performance results.

Education/Experience Required

  • Graduate of an accredited medical school, completion of a three (3) year U.S. residency program, and board certified in their specialty.
  • Minimum seven (7) years of clinical practice as a physician.
  • Previous performance-based leadership experience preferred.

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Qualifications

To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.

Basic Requirements

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
  • Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine
  • The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed
  • Current – full and unrestricted license to practice medicine or surgery in a State, Territory, Commonwealth of the United States, or in the District of Columbia
  • Residency Training: Physicians must have completed residency training approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification
  • Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME); (2) Those approved by the American Osteopathic Association (AOA); or (3) Other residencies (non‑US residency training programs followed by a minimum of five years of verified practice in the United States) deemed to have provided the applicant with appropriate professional training and exposure to a range of patient care experiences by the local Medical Staff Executive Committee
  • Residents currently enrolled in ACGME/AOA accredited residency training programs and who otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs)
  • PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee‑basis
  • PRPs are not considered independent practitioners and will not be privileged
  • They are to have a "scope of practice" that allows them to perform certain restricted duties under supervision
  • Additionally, surgery residents in gap years may also be appointed as PRPs
  • Proficiency in spoken and written English
  • Preferred Experience: Board certified and/or have completed residency in Internal Medicine or Family Practice/Family Medicine
  • Two years experience in outpatient primary care
  • Experience with outpatient clinical precepting such as with medical students and residents

Duties

  • Primary Care Physicians within Ambulatory Care will be responsible for the performance of and documentation of health care delivery for Veterans according to VHA and Durham Health Care System standards
  • This physician will be fully qualified and certified to function in both a clinical and administrative role in accordance with the guidelines and policies of the Durham Veterans Affairs Health Care System
  • This individual is delegated authority and responsibility in his/her area of expertise to assume major clinical, consultative, educational, research, and administrative duties related to Veteran care
  • Duties include but are not limited to: Clinical – primary focus of practice is the independent clinical evaluation of Veterans through their panel of patients including prevention and patient self‑management
  • Comprehensive primary care will be provided to male and female Veterans
  • As a gap provider – may cover short or long term leave of other primary care providers by seeing their assigned panel of patients
  • May be assigned to one panel for short or long term
  • May also cover same‑day provider call‑outs
  • May see patients with same‑day needs – hospital follow‑up or patients when their assigned provider is not available
  • May cover messages or computer 'alerts' for other providers at the same or another site within the Durham VA Health Care System
  • Comprehensive primary care includes but is not limited to evaluating patients in an outpatient clinical setting to diagnose and treat acute and chronic diseases, educating on health promotion and disease prevention, and ensuring health maintenance is up to date
  • Completes physical examinations – which may be comprehensive or focused to formulate treatment plans
  • Treatment plans are evidence‑based and engage in shared medical decision‑making
  • Maintains clinical knowledge and skills to provide comprehensive primary care services to male and female patients
  • Care may be provided in many modalities – from direct face‑to‑face care to virtual care (including telephonic, video, secured messaging, and others)
  • Promotes, directs, evaluates, and is responsible for referrals of clinical concerns found during evaluations
  • Functions as a member of a Patient Aligned Care Team – collaborating with clinical and administrative support to provide timely, quality, coordinated care for Veterans
  • Utilizes and promotes approved standards of care, procedures, methods, and techniques, safety precautions and protocols in all settings
  • Collaborates with other health care resources to ensure continuity of care
  • Keeps abreast of the latest trends, skills, procedures, and protocols relevant to primary care
  • Participates in continuous medical education per requirements (including opioid prescribing and trainings to fulfill and maintain designated women’s health primary care provider)
