Job Summary and Qualifications

As a Registered Nurse, you will be responsible for delivering high-quality, patient-centered care in line with the requirements of the department and the standards of practice for the relevant state and specialty. Collaborating with medical providers and the care team, you will provide personalized, comprehensive, and compassionate care, following established nursing models such as "Assess, Perform, Teach, and Manage." You will also act as an advocate for patients, families, and caregivers, embodying the organization's vision, mission, and values to ensure an outstanding patient experience and positive clinical outcomes.

What you will do in this role:

  • Assess the patient's condition during admission and each scheduled shift, promptly identifying and reporting any changes in patient status.
  • Perform procedures, monitoring, or other functions as ordered by the medical provider(s), and ensure thorough and timely documentation of care administration in the patient's medical record. - Administer prescribed medications, monitor the patient for therapeutic response, and take appropriate action in the event of an unintended response to the medication.
  • Provide exceptional care by responding promptly to patient requests, proactively anticipating patient needs, and resolving them.
  • Educate patients, families, and caregivers about the patient's medical condition, treatment plan, medications, possible side effects, and follow-up measures, ensuring complete understanding by translating complex medical terminology.

Job Summary and Qualifications

As the Pediatric Intensive Care Unit (PICU) Registered Nurse (RN), you will be responsible for performing patient assessments, developing a plan of care, and implementing and evaluating care. As a member of the interdisciplinary care team, you will work collaboratively with the other members to assure the safe and accurate delivery of care.

* You will perform and document patient assessments at the time of the admission

* You will create a plan of care, including discharge plans, revise the plans based on the patients’ response to treatment and regularly evaluate plans for effectiveness

* You will document the outcomes of nursing interventions and patient responses

* You will ensure physician orders are noted and correctly transcribed

* You will provide educational materials to patients and families on care and treatments

* You will keep physicians and supervisors aware of significant information regarding the patient and family

* You will administer medications and complete treatments according to hospital policy

* You will delegate work appropriately to non-licensed staff in accordance with standards of practice

What qualifications you will need:

Experience

Program Details

  • During the course of the 32-week period, the employee is required to work two (2) holiday shifts (defined by Department Manager or designee), if holidays fall during their temporary assignment period.

Job Description

RN will float to Eastern CT HHC Urology Practices as needed, with Waterford being the main base location.

Job Summary and Qualifications

    Occupational Therapist (OT), or Speech-Language Pathologist (SLP)

    Education

    Bachelor’s degree in nursing, master’s degree in PT,

    OT, SLP from an accredited educational program

    Experience

    2 years in certified home health

    Experience

    OASIS Certification and/or Quality/Compliance

    Certification

     

    3 years in certified home health

    Homecare Homebase experience

    Licensure, Certification, Registration

    Current CT Licensure in one of the specialty fields: RN or PT or OT or SLP in good standing

    Licensure, Certification, Registration

     

    Language Skills

    English verbal and written communication skills, including computer usage and teaching skills

    Knowledge, Skills and Ability Requirements   

     

    •       Experience in providing data analysis and statistical processes in quality reporting 

    •       Ability to design and develop performance measures 

    •       Experience in providing education 

    •       Experience in facilitation of PI teams using quality improvement methodology 

    •       Ability to effectively communicate both verbally and in writing 

    •

    Ability and willingness to travel to branches (regions) as needed

    •

    Travel within the Unites States may be necessary

    •

    Knowledge of federal and state regulatory requirements 

    •

    Excellent written and verbal communication skills 

    •
